Free physicals are available to Vancouver Public Schools students entering sixth through 12th grades in the upcoming school year.
No appointment is necessary on this date.
- Students whose last name starts with A through K will be seen from 1 p.m. to 2:30 p.m.
- Students whose last name starts with L through Z will be seen from 3 p.m. to 4:45 p.m.
What to bring/wear
- WIAA sports physical form with the medical history portion already completed
- Signed permission slip if student is not accompanied by a parent or guardian
- Wear loose-fitting shorts and T-shirts
Students with known cardiac, asthma, diabetes and/or seizure-related issues or other conditions of concern cannot be served. If an abnormality is found, the student will be referred to a primary care provider before being cleared for the sport.
Some children may be eligible for free or low-cost medical, dental and vision coverage through Washington state’s Apple Health for Kids. You can find out more information and/or apply at the physical. If you would like to apply, please bring proof of household income, birth certificates for the child(ren) who will be covered and picture IDs for children 16 years or older.
These physicals are provided by volunteers and are meant to target those athletes with difficulty accessing health care or those who do not have health insurance. These exams are not meant to replace regular health visits with your primary care provider. If you have access to primary care, please have your sports physical performed there.
